Beginning July 28, 2026, all Rec Well members and program participants must complete a one-time Assumption of Risk form through the Rec Well Member Portal. This requirement applies to all UW–Madison students, affiliate members, community members, skate pass holders, Nielsen Tennis Stadium members, and program participants.

Why is this change happening?

The Assumption of Risk requirement is being implemented as part of a risk management strategy directed by the State of Wisconsin in collaboration with UW–Madison and the Universities of Wisconsin Offices of Risk Management. This is a common practice at many recreation centers and higher education institutions, including other Universities of Wisconsin campuses.

What do I need to do?

To maintain access to Rec Well facilities and programs:

• Log in to the Rec Well Member Portal.
• Complete the Assumption of Risk form.
• Submit the form.

The form takes only a few minutes to complete and only needs to be completed once.

Here’s What You’ll Do:

Go to www.recwell.wisc.edu and select “Sign Up”  from the upper right corner.

This will take you to the Rec Well Member Portal. Select “Sign In” from the upper right corner.

After you select “Sign In” in the Upper Right Corner,

If you are actively employed at the University or you are a UW-Madison student, you will sign in with your NetID.

Otherwise, you will use your email or username to sign-in.

After logging in, you MAY have to click the notification bell in the upper right corner to access the waiver.

Will I still need to complete waivers for specific programs?

Possibly. Some Rec Well programs have additional risks, operate outside of Rec Well facilities, or have other unique requirements that may require a separate Assumption of Risk or waiver during the registration process. Completing the new Assumption of Risk form does not replace any additional program-specific waivers that may be required. 

What happens if I don’t complete the form?

Starting July 28, members who have not completed the Assumption of Risk form will not be able to access Rec Well facilities or register for programs until the form is completed. 

The good news is that access is restored immediately after the form is submitted.

To avoid any interruption to your access, we encourage you to complete the form as soon as possible.
