Audio, Visual, Scoreboard, & Technology Issues
Use this form for any audio, visual, or technology issues, including: sound system, projectors, screens, TVs, digital signage, room cards, scoreboards, turnstiles, WiFi/Internet, and phones.
Please provide as much detail as possible.
Audio, Visual, and other Technology New Project or Enhancement Request
This form is for audio, visual, lighting, or other technology requests that are new or are enhancements to the current technology in the facilities. Examples include: adding colored lighting to a new room, adding new digital signage, requesting a new audio input, wanting to have a dropdown screen and projector to a gym, etc.
This request is not for new computers, new tablets or iPads, new work stations, fixing computers, downloading software to a computer, iPads or tablets. Please talk to AIMS or your supervisor about those items.
Blended Learning
These trainings are due within 30 days of students starting or returning to Rec Well. All professional staff that supervise students were added as collaborators to the Qualtrics forms. If you do not have access please notify Mackenzie Lucius. Each area is responsible for distributing and tracking each training.
A few notes about the trainings:
- Student staff receive 1 hour/training at their positional wage. For example, new hires should receive 2 hours of positional pay after completing both trainings.
- Student staff will need to complete each training in one attempt. They are unable to exit the training and continue at another time.
Onboarding (NEW STAFF ONLY): This training includes Risk Management and E.I.D.
Leadership Development (NEW STAFF and RETURNING STAFF): This training includes Rec Well 101 and a Leadership Development section.

COVID Testing
UHS is offering testing on campus for UW-Madison students and employees. Once your profile is updated and you’ve filled out the proper consent forms, you can make an appointment. Learn more about testing here.
If you’ve tested elsewhere, such as Alliant Energy Center, UW Health, an urgent care center or a doctor outside of University Health Services, you should report positive test results to UHS using the secure email address: covidresults@wisc.edu or at 608-890-0000 (you will be provided prompts to follow). Results can also be submitted through a web form by logging into MyUHS, navigating to the Messages link, selecting New Message, and then choosing COVID-19 Outside Result Reporting.
Expense Tracking
You should complete and submit this form after making a purchase to ensure it will be accurately recorded into Great Plains.

- It allows us to track the divisional goal that every professional take advantage of one professional development opportunity a year.
- It allows for you/your supervisor to receive an email receipt of the form for your records and continued conversation.
- It allows for you to provide insight into the opportunity and whether it would be a good option for others in the division.
